Hospice Aide

This position provides personal care, supportive services, education, and light household duties according to a plan of care; communicates and tracks changes to reflect assigned duties; and provides patients the attention needed to create a sanitary and orderly environment.

The ideal candidate will possess a high school diploma or GED; one to three months related experience and/or training and CNA certification; effective communication and interpersonal skills; and the ability to apply sound or good judgment to carry out instructions.

Volunteer Coordinator

This position promotes the use of volunteers as an intervention to support patient care needs; recruits, hires, trains, supervises, and retains volunteers to support patients’ administrative and care needs; and provides education about volunteer services and the role of volunteers to members of the IDT.

The ideal candidate will possess a bachelor’s degree and experience in social services, human services, resource management, or a related field; effective teaching, interpersonal, and organizational skills; knowledge of Microsoft Office programs; and presentation experience.

Social Worker

This position provides direct social wok services and counseling, facilitates placements, and monitors patient adjustment; completes patient assessments and reviews and explains hospice services; develops and utilizes community resources; and maintains working relationships with community agencies.

The ideal candidate will possess a master’s degree in social work from an accredited school of social work; effective communication and interpersonal skills; knowledge of Microsoft Office software; and an understanding and acceptance of hospice philosophy.
