Director Clinical Services

This position conducts inter-disciplinary team meetings and manages team members; oversees the development of the plan of care and direction of patient care activities; and supervises patient and family care by assessing appropriateness, continuity, service, and quality of care.

The ideal candidate will possess a bachelor degree in nursing and RN license; a minimum of three years in healthcare, preferably in a nursing facility, home health, or hospice setting; and experience in leadership, management, and with electronic medical record systems.

Executive Director

This position oversees the on-site clinical, sales, and operations teams; develops the annual budget and monitors expenditures; ensures continued program growth by developing referral sources and vendors; and partners with clinical resources to oversee patient intake and inquiries.

The ideal candidate will possess a bachelor’s degree in healthcare administration, business administration, or a related discipline; two years of experience in the healthcare industry; five years of supervisory experience; and effective leadership, organizational, and interpersonal skills.

Registered Nurse Case Manager

This position coordinates clinical, psycho-social, and spiritual services as indicated by the plan of care; oversees the care given by LPN/LVNs and Hospice Aides; and provides physical and emotional intensive care to support patients and families in periods of crisis.

The ideal candidate will possess a Pennsylvania RN license and a minimum of one year of nursing practice or equivalent experience; effective communication and compassionate interpersonal skills; and strong leadership and organizational skills.
