Delaware County Memorial Hospital, located in Drexel Hill and one of the founding hospitals of the Crozer-Keystone Health System, is a 168-bed, not-for-profit facility that offers a broad range of acute and specialized services. Established in 1927, DCMH currently admits over 10,000 patients, treats nearly 40,000 Emergency Department patients, completes more than 5,800 surgeries and, delivers more than 1,800 babies each year.

PPS/Rehab Care Coordinator

This position provides coordination for the timely and accurate completion of the Patient Assessment Instrument for each rehabilitation inpatient. This individual provides clinical support in completion of data contained in the PAI for each patient to ensure accuracy in the scoring of functional status and reflection of the patient’s care needs. He/she plans and coordinates care of the patient from pre-hospitalization through discharge. The person also coordinates the discharge planning function in conjunction with the social worker and admissions coordinator, and assists with screening, investigation, and problem solving as needed.

The ideal candidate will possess a PA License as either a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Therapist. He/she should have three to five years of experience in the care of patients in an acute hospital or rehabilitation setting. The person must have knowledge of a basic computer and data management software, with excellent communication and organizational skills.

Physical Therapist

Candidates must have a PA Physical Therapist License, and either a Bachelor’s or Master’s Degree in Physical Therapy. He/she should have one year of general experience within the field and additional training with emphasis in acute care/rehab. One year of experience in Home Care is preferred.

Clinical Director

This position will manage the human and material resources of the Perioperative Departments. He/she will provide quality patient care and an environment conducive to staff growth and satisfaction. The person must have current licensure to practice Professional Nursing in the Commonwealth of Pennsylvania. The shift is from 7 AM-3 PM.

The ideal candidate will have a Bachelor’s Degree in Nursing, as well as a Master’s Degree. He/she should have three years of relevant clinical experience, and must have management experience.
