Full-Time LPN

Dunwoody Village is currently seeking dedicated and caring Licensed Practical Nurses (LPNs) to join its team for a full-time 3 PM to 11:30 PM position.

Dunwoody Village has new starting rates for all positions, and it pays for experience. This position is eligible for a $10,000 sign-on bonus paid during their first year of employment.

Qualified candidates must have at least 2 years of experience as an LPN in Long Term Care, Rehab, Memory Care, and Skilled Nursing. All applicants interested in full-time must be available to work within a weekend and holiday rotation.

Primary nursing duties include:

  • Provide exceptional resident care according to the Plan of Care
  • Provide a safe environment for our residents
  • Comply with all documentation and record-keeping requirements (utilizing Point Click Care)
  • Possess excellent written and verbal communication, documentation, and computer skills

Personal Care and Homecare Administrator

Dunwoody Village is seeking a dedicated, enthusiastic, and intrinsically motivated leader to manage its Personal Care and Home Care programs.

This position will be responsible for managing all aspects of the personal care + memory support units in the Dunwoody Village Care Center as well as the Dunwoody at Home home care program.

This is a unique opportunity to make dramatic and positive changes in the lives of others.

The Personal Care / Home Care Administrator is responsible for the daily operations and care provided to the personal care and memory support residents through directing and evaluating unit activities to ensure safe and effective practices.

This administrator ensures compliance with all regulations and develops, revises, and implements policies and procedures for personal care services and activities.

This administrator acts as a problem-solver and liaison between residents, designated person(s), and staff members.

This administrator coordinates support plan meetings and manages the admission process in designated areas.

Successful candidates will possess the following knowledge, skills, and abilities:

  • Current credentials and at least 3 years of experience as a PA Personal Care Administrator.
  • High degree of resident assessment and management skills; exceptional organizational and time management skills.
  • Demonstrated experience with Point Click Care (PCC) or similar medical records systems.
  • The intrinsic ability to successfully lead and motivate a team of dedicated and talented healthcare professionals.
  • Keen sense of detail and desire to be “survey ready” at all times.
  • Plays an active role in developing Personal Care and Home Care through team building, coaching, mentoring, and performance management.
  • As a member of an interdisciplinary team, helps to provide training to home care and personal care staff to effectively provide resident care as outlined in respective job descriptions.
  • Develops standards of quality, clinical excellence, resident safety, resident satisfaction, and staff engagement.
  • Develops reporting metrics on quality standards and satisfaction levels and publishes results on a quarterly basis. Takes appropriate action to correct any deficiencies in quality and satisfaction identified in such reports.
  • Conducts ongoing meetings to review resident assessments and other resident status changes and updates with all stakeholders.
  • Manages and reviews records and charts of residents/clients for pertinent information.
  • Manages and reviews staff documentation to ensure a comprehensive and compliant medical records program is in place and followed.
  • Develops both short-term and long-term strategic plans to ensure overall growth and development of personal care and home care programs.
  • Helps to prepare and manage annual functional budgets to ensure effective and efficient financial stewardship of Dunwoody resources.
  • Develops appropriate staffing levels and ratios. Provides guidance as needed to scheduler to ensure staffing levels are met.  Ensures that staffing levels are appropriate based upon a variable resident census.
  • Maintains compliance with federal regulations, state regulations, local regulations, and Dunwoody Village policies and procedures.
  • Completes and submits required reports, including, but not limited to, responses to PA Department of Human Services inspections, etc., ahead of any required deadlines where applicable.
  • Stays current with the industry and reports on trends as appropriate.
  • Provides ongoing and timely communication on resident status with families and/or resident support systems to promote positive and informed relationships.
  • Directs the clinical operations and supervisory activities of supervisors and evaluates adherence to performance standards.
  • Develops a schedule of staff educational opportunities, including in-services, equipment/technology training, continuing education, etc.
  • Maintains confidentiality of all resident and staff personal or health information.


Our successful candidate must have experience with age-specific competencies of the geriatric population in a long-term care setting.

Dunwoody Village recently adopted a Montessori-based resident care program, and relevant experience is highly desirable (or demonstrated willingness to lead Personal Care in this regard).

This person should also have strong computer skills with the ability to use Microsoft Office programs. The ability to positively communicate with people of diverse backgrounds and educational levels is paramount.

Part-Time Clinical Managed Care Nurse Case Manager

Dunwoody Village has a rare and unique part-time opportunity for an experienced RNAC/Managed Care Nurse Case Manager due to the pending retirement of one of its long-serving RNACs. 

The care center is made up of 81 skilled care rooms and 81 personal care rooms, and the RNAC serves an integral role in ensuring that operations run smoothly and efficiently.

Some of the key essential functions of the position include the following:

  • Conduct and coordinate the development and accurate completion of the RAI and care planning process, including the implementation of CAA’s.
  • Performs admission chart review and has knowledge of ICD 10 coding and PDPM reimbursement process.
  • Coordinates scheduling of OBRA and Medicare/managed care assessments, entry/re-entry of records, and care plan reviews in accordance with the most recent regulatory guidelines.
  • Provide regular, timely clinical updates to Managed Care insurance organizations.
  • Assists the Director of Nursing to ensure all documentation in the clinical record meets federal, state, and certification guidelines.
  • Coordinates scheduling of care plan conferences following the RAI manual and any applicable state guidelines.
  • Work with staff to enhance reimbursement, as applicable, through documentation of care and services provided.
  • Work as part of the overall team to ensure documentation of care and services is appropriate and supports reimbursement.
  • Inform residents, responsible parties, and staff of Medicare/managed care coverage status through appropriate documentation in accordance with all federal, state, and organization guidelines.
  • Adheres to the established organization policies related to infection control and reducing the potential spread of infection.
  • Works cooperatively as part of the health care team to evaluate and revise current practices, policies, and procedures.
  • Carry out QAPI roles and responsibilities as assigned
  • Communicate to leadership upon witnessing a positive outcome of a QAPI project or detecting barriers preventing project success.
  • Support QAPI efforts both verbally and non-verbally (i.e., via actions and attitude), including adjusting performance and practice in accordance with QAPI initiatives and findings.
  • Understands, complies with, and promotes all rules regarding resident rights and confidentiality.
  • Attends and participates in orientations, training programs, educational activities, in-service functions, and staff meetings.
  • The RNAC must possess appropriate assessment and documentation skills to support the PDPM model.

The ideal candidate has:

•A minimum of 3 years of RNAC experience preferred – ideally in the CCRC setting.

•Superb clinical and administrative skills with a detail-oriented approach to completing tasks.

•Working knowledge of Point Click Care and Microsoft Office – including Word and Outlook.

Proficiency or experience in ICD-10 coding

•Ability and keen desire to work effectively in a team-based setting.

•Willingness/desire to provide exemplary customer service to both internal and external customers.
